This makes working with complexly structured documents easy
and more reliable as the parser is not susceptible to
element re-ordering.
Also fixes a bug when the tokenizer would choke after reading
a number.

This patch removes includes that are not used. The removals were found with
include-what-you-use which checks if any of the symbols from a header is
in use.
